A Program Advocate will begin an intake to determine what services are needed.
Once services are determined that Program Advocate will begin research on what resources are available in the individual’s community.
The Program Advocate will then:
- meet with the individual (location of their choosing) to discuss the resources that are available
- Ensure that the individual has an appointment scheduled and transportation to the appointment
- Help complete any paperwork or online communication required
- Follow up with the individual weekly for two weeks and then monthly for six months to ensure no more resources are needed
