Pre-admission Screening (PAS)



PAS screens individuals to determine their eligibility for DD (Developmental Disability) services. Once an individual is found eligible, PAS then requests funding for the desired services and seeks the services needed. Services needed could include Home Based Services, Residential Services in an ICF/DD or a 24 hour CILA. PAS also determines the need for a nursing facility, whether temporary or permanent placement. Individuals screened must be Medicaid eligible and diagnosed with a developmental disability prior to the age of 18.

Waiver Case Coordination



PSCI staff spends time developing relationships with individuals with developmental disabilities and their families. Building these relationships helps staff advocate for the individuals and families served to ensure they are receiving the services they desire to help them reach their dreams and maintain their safety and wellbeing. PSCI staff advocates for employment opportunities, desired daytime activities such as day training programs for individuals with developmental disabilities and desired residential settings.

Case Coordination / Case Management



Independent Service Coordinators “ISC’s” meet with individuals and/or guardians to discuss options and programming offered through the Illinois Department of Human Services. Services are chosen based on the desires and needs of the individual. If the individual does not currently receive Social Security or Medicaid benefits, the ISC guides them on how to obtain those services. After eligibility for DD services is determined, ISCs meet with individuals/guardians again to discuss options and agencies. ISCs then set up tours and send out referral packets to provider agencies that have been chosen by the individual and/or guardian. Often times, ISCs will accompany individuals/guardians on the tours and to any meetings with agencies. Case Coordination/Case Management is provided for individuals on an ongoing, as needed, basis. If an individual does not qualify for DD services, they are referred to the appropriate agency that provides the services needed.

Bogard Services



Bogard serves individuals who have a developmental disability and resided in a nursing home between March 23, 1986 through April 1, 1994 for 120 days or more. Because of a consent decree, these individuals became Bogard Class Members and are entitled to Specialized Services.

PUNS



PUNS (Illinois Prioritization of Urgency of Need for Services) is a statewide database that records information about individuals with developmental disabilities who are potentially in need of services. The state uses the data from the PUNS to select individuals for services as funding becomes available. Completion of the PUNS does not guarantee that an individual will receive supports funded by the Division of Developmental Disabilities through the Illinois Department of Human Services, nor does it determine their eligibility for such supports.
